Our office is conveniently located directly off Highway 422 on Route 306 in the Promptcare/Keybank building in Suite 205. Our address is 17747 Chillicothe Rd., Chagrin Falls, OH  44023.  Handicap parking is available in the rear of the building.  Our normal office hours are: Monday and Wednesday 7:30am - 5:00pm, Tuesday and Thursday 7:30am - 6:00pm and Friday 9:00am - 3:00pm. 

Emergency appointments are available daily.
